Center for Loss & Hope
Losing a person you love is one of lifeās most difficult challenges. The grieving process can include feelings of pain, isolation, depression, confusion, anger and resentment. Sharing grief is one of the best ways to heal. YoloCares offers a wide range of support programs for adults, teens and children at no cost.

Center for Caregiver Support
Thanks to a deeply generous gift from the Arthur N. Rupe Foundation, the newly created Center for Caregiver Support will provide multi-faceted assistance and education to Yolo County caregivers, many of whom are seniors, facing the challenge of caring for a seriously ill loved one at home.

YoloCares Life Transitions Project
Mission Statement: Every person deserves access to high quality end-of-life care, but for rural and indigenous communities access is often compromised by cultural, geographic and institutional barriers. Through the Life Transitions Project, YoloCares is deconstructing obstacles to ensure equal access for all. Guided by the spirit of community empowerment, YoloCares recognizes that our diversity is what makes us whole.

Advance Care Planning
Advance Care Planning allows people to have more control over the kind of medical care they want and donāt want if they become seriously ill. However, only about 30% of Americans have made their wishes known by completing an advance directive for health care.

YoloCares University
YoloCares is proud to offer YoloCares University, an educational resource for clinicians and employees of partner facilities. YoloCares University will host opportunities for clinicians to obtain Continuing Education Units (CEUs) and will serve as a hub of learning for the local healthcare community.
