Palliative Medicine & Supportive Care
Palliative care is specialized and highly coordinated care that improves your quality of life while your physicians work to cure or manage your condition. We collaborate with your doctors on a treatment plan that supports you and your goals. And we administer care wherever you are, whether at home or in a hospital.
The human spirit is at the heart of what we do. Even when the end of life appears on the horizon, there is always more that can be done. Exceptional care begins with understanding your goals. It begins with us listening to you . . . how you feel and what’s important to you.

Center for Loss & Hope
Losing a person you love is one of life‘s most difficult challenges. The grieving process can include feelings of pain, isolation, depression, confusion, anger and resentment. Sharing grief is one of the best ways to heal. YoloCares offers a wide range of support programs for adults, teens and children at no cost.
Center for Caregiver Support
Thanks to a deeply generous gift from the Arthur N. Rupe Foundation, the newly created Center for Caregiver Support will provide multi-faceted assistance and education to Yolo County caregivers, many of whom are seniors, facing the challenge of caring for a seriously ill loved one at home.
YoloCares Life Transitions Project
Mission Statement: Every person deserves access to high quality end-of-life care, but for rural and indigenous communities access is often compromised by cultural, geographic and institutional barriers. Through the Life Transitions Project, YoloCares is deconstructing obstacles to ensure equal access for all. Guided by the spirit of community empowerment, YoloCares recognizes that our diversity is what makes us whole.
Advance Care Planning
Advance Care Planning allows people to have more control over the kind of medical care they want and don’t want if they become seriously ill. However, only about 30% of Americans have made their wishes known by completing an advance directive for health care.
YoloCares is proud to offer YoloCares University, an educational resource for clinicians and employees of partner facilities. YoloCares University will host opportunities for clinicians to obtain Continuing Education Units (CEUs) and will serve as a hub of learning for the local healthcare community.
